The oral Health Savings Plan Offers:

  • No Pre-Existing Conditions Limitations

  • No Waiting Periods

  • No Restrictions – Including cosmetic services such as teeth whitening, tooth colored restorations and veneers plus any other services offered at Maplewood dental.

  • 5% Savings with Automatic Renewal

  • 2 Fluoride Treatments

  • Unlimited Necessary X-rays

  • Unlimited Necessary Exams

  • 15% Discount on All Other Services

Oral Health Savings Plan

All savings are at our Maplewood Dental office only.  We reserve the right to refer certain procedures to the appropriate specialists.  OHSP discounts do not apply to dental services provided by a dentist other than those at Maplewood Dental

  • The OHSP is Non-refundable and non-transferable.

  • The OHSP will automatically renew with a 5% discount on next paid membership.

  • Membership fees will be reviewed and annually and are subject to change without notice.

  • Procedural fees are at the discretion of Maplewood Dental.  All Payments are due at the time of service.
